Other issues are:
-how do you define locally advanced disease?
-What is the definitive treatment for that: CRT vs RT vs surgery.
-How do you define resectable?
-Do they get adjuvant therapy and if so, what?

These questions explain why agreement is tough......ie no-one can figure it out. Thus the individual pt discussions.

And perhaps most troubling to me is: what about the concept of 'converting' to locally advanced?
I was always taught that regardless of response, the surgery is the same.
