Update Template

This template update adds the below items. It attempts to mimic current godoc.org package template.

• Produce slightly less whitespace for the markdown parser to deal with
• Add index
• Fix non-existant links
• Add functioning anchor links
• Add source file sections
• Add example sections (there appears to be a bug here unrelated to the template)

thanks. I have no unit tests in this repo, but I trust you've tested this.

@davecheney I've rendered several hundred pages so far without issue. I should note though, that I've provided my own version of this heavily inspired by yours (thank you very much). The one thing to note is that the currently non-functioning -links flag will not be honored and anchor links and source links (which wont work right) will be rendered wether the user likes it or not.

IMHO, it is an improvement. If users don't like the anchor linked index, they should submit a PR to fix it :)
