Refactor completely tagging support and fix #3

Now, \etocname, \etocnumber and \etocpage will individually hold the
tagging (inclusive of hyperlinking) support.

Add \etoctagginginlineoff to make it possible for line styles to use
\l@section or \@dottedtocline themselves as per the documentation
section Another compatibility mode (issue #3).

Add test files for #1, #2, #3.
