ElvUI Better Totem Bar

The idea is to develop a Totem Bar replacement for ElvUI inspired by FloTotemBar.

1.0.0 is the first kind of stable release, I'll be working in fixing all the bugs before the release of MoP.

• The cooldown for transformed spells like Earthgrab Totem are not working correctly.
