Prep for 1.0.0 release

 - Moves components so that they can be required externally with e.g.
   require('velocity-react/velocity-transition-group')

 - Fleshes out README by including documentation from the source files.

 - Adds clarity on how to require UI Pack, and avoids doing it by
   default.

 - Changes Velocity dependency to 1.2.3, which includes our finishAll.
   Also makes the React peerDependency less specific.

 - Includes the missing css-loader dependency.

 - Adds a convenience entrypoint.

Also includes fixes to the crossfade example and component that were
mistakenly left out of the final merge.
