Cleanup, added try/catch statement

Added try/catch as part of ActiveXObject creation (thanks to
github.com/mcfedr for the suggestion)

Some minor cleanup work:
Cleaned up whitespace to make Crockford happy
Revised JSLint directives to make Crockford happy
Moved all variable declarations to the top of the function to make
Crockford happy
